Erin says ABSOLUTELY. I THINK A MAJOR PROBLEM IS ALSO HOW WE TEACH KIDS TO EAT WHEN THEY'RE BABIES. MOMS ARE OKAY, THAT'S THE FOOD ON YOUR LATE AND YOU MUST EAT THE AMOUNT OF FOOD PUT THERE AND IN MY EXPERIENCE AS A MOM AND AS A DOCTOR, KIDS CAN REGULATE THEMSELVES VERY WELL. Daniel says SLULTLY IS. BIG TIME. IT REALLY HURTS ME TO SEE A KID OVERWEIGHT YOU GO INTO A MALL AND THEY ARE EVERYWHERE. I OBVIOUSLY SEE THEM AND I THINK IT'S TEAR IVELT IT WOULDN'T BE VAING TO HAVE HEART ATTACKS 35RX 40 YEARS OLD. I THINK 50 IS YOUNG, WAY TOO YOUNG TO BE HAVING HEART ATTACKS.

Mary says WHAT WOULD BE SOME OF THE BARRIERS THAT MIGHT STOP OVERWEIGHT KIDS FROM WORKING OUT?

Daniel says IT'S YOU WANT TO PARENT TO TEACH THED KID A HEALTHY LIFESTYLE AT A YOUNG AGE. THAT GOES FOR BEING ACTIVE, PLAYING SPORTS. A LOT OF PEOPLE SAY MY KID WITH A PERSONAL TRAINERS, THAT'S CRAZY. BUT THERE'S A LOT OF POINTS WHERE YOU MIGHT BE AND YOU CAN NEED A PERSONAL TRAINER. YOU GET THIS VISION, THAT'S CRAZY, IT'S FOR RICH PEOPLE, I CAN'T AFFORD. IT'S NOT ABOUT THE PERSONAL TRAINER, IT'S ABOUT THE KID COMING TO SEE US AS A FRIEND, HELPING THEM BECOME THAT KID WITH MORE CONFIDENCE. THEY DO GET MADE FUN OF, KIDS ARE KIDS AND THEY JUST DON'T SUNDAY AND I THINK THAT'S WHERE IT'S IMPORTANT. WE CAN HELP, WHEN KIDS COME SEE US. WE'RE NOT THEIR TEACHERS, THEIR COACH, WE'RE NOT GOING TO BENCH THEM. IT'S NOT A SPORT. I THINK TEACHING PEOPLE TO EAT HEALTHY IS ONE OF THE MOST IMPORTANT THINGS YOU CAN DO FOR HIM AND IN FACT AT THE AGE OF 16 HE STILL MAY BE GLOWING HEIGHT EVEN THOUGH HE IS A BIG BOY. SO THE FIRST PRINCIPLE IS TO TEACH HIM PATTERNS OF EATING WITH. THE SECOND THING IF HE'S JUST GOING A GYM YOU HAVE TO BE REALLY CAREFUL ABOUT AN UNSUPERVISED WEIGHT TRAINING PROGRAMME. HE NEEDS A COMBINATION OF AEROBIC, FIFTHNESS, COMBINATION OF WORKING ON A BICYCLE OR TREADMILL AND MOTT JUST WEIGHT TRAINING SELF. AND WHAT YOU WILL NOTICE IS HIS WEIGHT MAY NOT CHANGE DRAMATICALLY. MUSCLE WAIST MORE THAN FAT. YOU'LL NOTICE HIS PANTS WILL FIT DIFFERENTLY AND THAT HE'S EEL JUST START FEELING BETTER. THE WEIGHT SHOULDN'T BE THE ISSUE. IT IS REALLY HOW HE FEELS. THAT I WOULD FOCUS ON. Mary says YES, YOU'RE SAYING IT IS A PROGRESSIVE ILLNESS, IS IT COMMON FOR CHILDREN NOT TO HAVE SEVERE SYMPTOMS? IS THIS A COMMON THING?

Neil says WELL I THINK THE POINT TO BE MADE IS IT'S PROGRESSIVE AND SLOWLY PROGRESSIVE SO THAT IT'S QUITE COMMON NOWADAYS THAT IF YOU COME TOMORROW OUR C.F. CHIN ANYONE, IN A ROUTINE VISIT, YOU WILL SEE HAPPY HEALTH WHERE LOOKING KIDS COME IN, HAPPY HEALTHY LOOKING KIDS GO OUT. THAT HAPPENS A LOT.

Neil says WELL IN TIMES GONE BY IT DID NOT, SO OVER HISTORY, EVEN THE LAST COUPLE OF DECADES THINGS ARE CONTINUING TO IMPROVE.

Mary says HOW ARE WE ABLE TO DO THIS?

Neil says WELL, I THINK BASICALLY THERE HAVE BEEN IMPROVED TREATMENT, YOU KNOW, RESULTS OF RESEARCH HAVE SUGGESTED BETTER AND BETTER WAYS OF DEALINGS DEALING WITH THE ILLNESS BUT TO BE HONEST, YES, OVER I'M AND AS THE KIDS GET OLDER, YES THEY'RE SOMETIMES HAVING A LITTLE TROUBLE BREATHING AND SOMETIMES THEY HAVE THESE THINGS WE CALL EXACERBATIONS WHERE THEY HAVE SORT I HAVE CUTE ATTACKS WHERE THEY TAKE A BIT OF A DIP. YOU KNOW, PEOPLE WHO HAVE ASTHMA COULD LOOK PRETTY NORMAL IN BETWEEN TIMES BUT WHEN THE ASTHMATIC HAS AN ATTACK, HE'S CLEARLY SICK. AND IN THE CASE OF CYSTIC FIBROSIS, EVEN THOUGH IT'S A BIT DIFFERENT AND A BIT MORE SEVERE, OFTEN, YOU CAN HAVE VERY GOOD PERIODS BUT OCCASIONALLY YOU CAN HAVE THESE EXACERBATIONS WHERE THEY ARE SICKER AND MAYBE NEED TO COME INTO HOSPITAL FOR TREATMENT FOR A WHILE.

Mary says AND WHAT ABOUT THE LIFE SPAN, AS WELL. HAS INCREASED?

Neil says QUITE A BIT EVEN WITHIN MY MEMORY. I REMEMBER IN 1980 WHEN I WAS AN INTERN AT SICK KIDS IT WAS UNFORTUNATELY QUITE COMMON TO SEE TEENAGERS DYING WITH SYSTEM FIBROSIS AND THE AVERAGE LIFE EXPECTANCY THEN WAS LATE TEENS. NOW, 23 YEARS LATER, WE'RE DEALING WITH AVERAGE LIFE SPAN RUNNING IN THE MID-30s. SO IN THAT PERIOD OF TIME, IT'S DOUBLED. AND CERTAINLY IT'S NOW VERY COMMON THAT OUR KIDS AT AGE 18 GRADUATE FROM THE SICK KIDS CYSTIC FIBROSIS CHIN DISMIK GO OVER TO St. MICHAEL'S HOSPITAL WHERE THE ADULT CLINIC IS NOW AND THEY'RE USUALLYLY IN REALLY VERY GOOD CONDITIONS, LOOKING FORWARD TO PLANNING THEIR LIVE, LOOKING FORWARD TO PERHAPS WORK OR PERHAPS MARRIAGE OR PERHAPS FURTHER EDUCATION SO THAT, YOU KNOW THINGS THAT IN THE PAST MIGHT HAVE BEEN HARD TO HOPE FOR ARE BECOME MUCH MORE COMMON NOW.

Mary says YOU KNOW, THAT'S WONDERFUL THAT THE LIFE SPAN HAS INCREASED SO MUCH. HALF OF ME, OF COURSE, IS THINKING. FROM THE POINT OF VIEW OF A MOTHER, AND KIM SITTING THERE, I HAVE TO ASK A QUESTION LIKE THAT ABOUT LIFE SPAN BUT EVEN WE YOU SAY 30s, THAT'S STILL A DIFFICULT ANSWER FOR YOU TO HEAR.

Kim says SHOOTLY THAT WOULD BE ME NOW, BASICALLY. AND YEAH, I MEAN THAT'S A DEVASTATING THING TO BE FIND OUT, TOO, WHEN YOU FIND OUT WHAT YOUR CHILD HAS THIS AND I THINK IT WAS THE FIRST QUESTION MY HUSBAND ASKED. AND YEAH, IT'S BIZARRE. BECAUSE WE DON'T ENTER PARENTHOOD THINKING AT ANY POINT IN TIME THAT WE ARE GOING TO OUTLIVE OUR CHILDREN SO THAT'S A SCARY THOUGHT.

Mary says SO HOW DO YOU APPROACH THE ILLNESS WITH YOUR DAUGHTER? SHE STILL IS ONLY FOUR AND A HALF, BUT YOU KNOW, HOW MUCH DO YOU TELL HER ABOUT? OR HOW MUCH DOES SHE EVEN KNOW?

Kim says SHE DOESN'T KNOW A LOT ABOUT CYSTIC FIBROSIS, PER SE. SHE KNOWS THAT IN HER WAY SHE'S SPECIAL AND SHE'S DIFFERENT. BUT WE SORT OF EMPHASIZE THAT AS BEING EVERY CHILD HAS THEIR OWN THING THAT MAKES THEM SPECIAL. WE DON'T REFER TO TO IT AS SYSTEM SEIVE STICKS FIBROSIS, BUT SHE KNOWS SHE HAS TO EAT CERTAIN WAY, GET PHYSIO THAIRNGS TAKE AN ENZYME BEFORE SHE EATS AND ALL OF THAT IS WHAT WILL KEEP HER A HEALTHY, ACTIVE CHILD. IT'S SORT OF WHAT WE EMPHASIZE.

Mary says SO DOES THAT ALL HAVE MUCH OF AN IMPACT ON HER DAILY LIFE?

Kim says IT HAS... IT DEFINITELY HAS AN IMPACT, BECAUSE BEFORE, YOU KNOW, AT THE BEGINNING OF ANY DAY, SHE HAS TO HAVE A MASK FOR 20 MINUTES AND BREATHE IN AN ANTIBIOTIC TO HELP CLEAR HER LUNGS OF BACTERIA. AND THEN SHE HAS TO HAVE ABOUT 35 MINUTES OF PHYSIOTHERAPY, AND THAT'S BEFORE SHE BEGINS HER DAY.

Mary says WHY PHYSIO SNAIRP WHAT DOES THAT DO?

Kim says IT'S A PREVENTIVE MEASURE TO KEEP HER LUNG, THE THICK MUSE KUK DISCRETIONS THAT MAY BUILD UP IN HER LUNG, IT JUST CLEARS THEM.

Mary says AND THAT IS A HALLMARK OF THE DISEASE.

Neil says YES. A STANDARD PROBLEM WITH SYSTEM FIBROSIS IS THAT THE SECRETIONS OF THE BODY THAT SHOULD BE WATERY AND FLOW SMOOTHLY TEND NOT TO BE. THEY TEND TO GET THICKER AND STICKIER AND ARE SO IN THE LUNG, FORCE, THE LUNGS CONTINUED TO PLUG UP AND THAT'S JUST A SET-UP FOR INFECTION. AND THEN THE INFECTION DOESN'T CLEAR WELL. SO THE PHYSIOTHERPAY OF WHICH WE SPEAK IS JUST A GENERAL TERM TO DESCRIBE ANY TREATMENT, ANY PHYSICAL TREATMENT THAT'S DONE TO HELP GET THE SECRETIONS OUT AND THE CLASSICAL SORT OF THING THAT PEOPLE TALK ABOUT IS WHERE SOMEONE DOES SOME CLAPPING ON THE CHEST IN SPECIAL POSITIONS TO HELP LOOSEN THE SECRETIONS AND BREATHING EXERCISES TO COUGH IT OUT.

Mary says WHAT ARE THE JANES OF TREATMENTS?

Neil says OKAY, ONE OF THE MOST IMPORTANT THINGS THAT WE'VE ALREADY MENTIONED IS THE ISSUE OF THE ENZYMES TO HELP THE DIGESTION. INITIALLY, ONE OF THE THING THINGS THAT HITS THE PERSON WHO HAS CYSTIC FIBROSIS IS USUALLY MY CHILD'S NOT GAINING WEIGHT WELL OR SOME OTHER INDICATION THAT THE ABILITY TO HANDLE NUTRIENTS AND FOR THE BOWLS TO MOVE PROPERLY IS NOT WORKING. AND SO BY GIVING THESE ARTIFICIAL ENZYMES, YOU ALLOW THE PATIENT TO DIGEST FOOD,EST AND SPLEEGS FAT IN WAY THAT OTHERWISE WOULD BE DIFFICULT OR IMPOSSIBLE FOR THE PATIENT. SO ENZYMES TO DIGEST FAT ARE A MAJOR PART OF TREATMENT. ALONG WITH THAT. YOU NEED EXTRA VITAMINS, BECAUSE THERE ARE THE VITAMINS THAT GO IN WITH THE FAT, VITAMINS A, D, E, AND K, AND IF YOU DON'T HAVE EXTRA AMOUNTS OF THAT, THE PATIENT CAN RUN SHORT OF THOSE VITAMINS. CERTAINLY TREATMENTS TO CLEAR SECRETIONS AND ANTIBIOTICS TO HELP DEAL WITH OR SUPPRESS LUNG INFECTIONS ARE VERY IMPORTANT. AND THEN AFTER THAT, A LOT OF THE TIME IT DEPENDS ON THE SPECIFIES OF THE IF I CAN PATIENT BUT AS AN EXAMPLE, IT'S VERY COMMON THAT MANY PATIENTS WITH CYSTIC FIBROSIS ALSO HAVE AN ELEMENT OF ASTHMA AS WELL SO IT'S VERY COMMON THAT WITH THE PHYSIOTHERAPY YOU'D ALSO BE BREATHING IN SOME ASTHMA MEDICATIONS TO HELP YOUR AIRWAY STAYS A OPEN AS POSSIBLE.

Mary says WE'RE ALMOST OUT OF TIME BUT YOU HAVE TO TALK ABOUT YOUR BOOKS AND THE SERIES. TELL US ABOUT THIS NEW ONE.
